The Blount County Community Health Initiative is seeking award nominations for its annual “Community Health Initiative Lifetime Achievement Award.” This award is presented to a distinguished individual or group that has made a long-term contribution to the advancement of the goals and objectives of the Blount County Community Health Initiative.
To qualify for this award, the individual or group must have shown leadership that has made a significant impact to improve the health and well-being of the community through increased awareness and promotion of prevention efforts; must have demonstrated outstanding initiative in their work that has positively impacted the community; must have displayed commitment to support prevention programs and projects that enhance health and wellness in the community; must have provided creativity in achieving prevention goals and objectives; and must have inspired others to greater involvement and participation in prevention efforts.
Nominations may be sent in letter format to the nominations committee at Community Health Initiative, 907 E. Lamar Alexander Parkway, Maryville, Tenn. 37804.
